Care Team Manager

The role

About the role

As Care Team Manager you run the day-to-day of our in-home and community support: who covers which shifts, how well care is delivered, and how the team is doing. It’s a hands-on leadership role in a small, close team — part people-leader, part coordinator, part support worker.

Day to day

What you’ll do

Lead the team

Supervise, coach and support our support workers — regular check-ins, performance and wellbeing.

Own the roster

Build and manage rosters in ShiftCare so every participant has reliable, consistent coverage.

Match & keep continuity

Pair workers to participants by needs, skills and rapport — and protect that continuity.

Onboard new staff

Induct new support workers and make sure they are trained and compliant before their first shift.

Uphold quality

Monitor progress notes, service delivery and incidents; hold the NDIS Code of Conduct.

Be the go-to

First point of contact for the team, participants and families — day-to-day questions and escalations.

Coordinate

Work with Support Coordination, admin and leadership on needs, referrals and service changes.

Stay hands-on

Carry a small direct support caseload and lead by example.

The fit

What we’re looking for

Essential

  • Experience in disability and/or aged care support
  • Team leadership, coordination or rostering experience (or strong readiness to step up)
  • Solid understanding of the NDIS and person-centred support
  • Strong communication, organisation and problem-solving
  • NDIS Worker Screening, Police Check, WWCC, First Aid/CPR (or willing to obtain)
  • Right to work in Australia and a current driver’s licence

Nice to have

  • Cert III/IV in Individual Support / Disability (or equivalent)
  • A background in nursing, physiotherapy or allied health
  • A bachelor’s degree in a health or related profession
