Support Coordination is an NDIS-funded support designed to help you understand and implement your NDIS plan, connect with the right providers and community services, and build the skills and confidence to manage your supports over time
At Amigo Personal Care, we work alongside you (and your nominated decision-makers where relevant) to turn your plan into real-life outcomes—focusing on your goals, your preferences, and what works best in your day-to-day routine.
Your Support Coordinator helps you make the most of your plan by linking you with NDIS providers and other community/government supports, and supporting you to develop the skills to coordinate services more independently.
We can support you to:
• Understand your plan and what it can be used for
• Identify and connect with suitable providers and services
• Coordinate appointments and supports in line with your goals
• Strengthen your capacity to manage services and navigate changes
• Help track plan budgets at a practical level (so supports remain sustainable)
